New Client Forms

Scheduling

We do not accept insurance at this time. We will provide you with an invoice and appropriate diagnostic codes, should you wish to submit to your insurance provider for reimbursement.

 

Payment is due at the time of service. 

Forms of payment accepted: cash, check, credit, debit

 

Once you schedule an appointment, we will email you new client forms including privacy practices and history/background questions to assist in a productive first session. 

 

Sessions are typically 50 minutes, but new client appointments are 60 minutes in length.
